Q1: What is Maxi Scheme?
A1: Maxi Scheme is an exclusive scheme designed to offer maximum value and protection for all the Liberty Insurance Berhad customers at the lowest possible rate. This scheme includes two PA plans, Maxi Rider PA Plan and Maxi Pro PA Plan. With any insurance plan purchases/purchased from Liberty Insurance Berhad, customers and their immediate family members are entitled to purchase either of these plans for at ONLY RM18 per annum.
Maxi Rider PA Plan is a motor vehicle driver and passenger insurance plan that protects the driver and/or passengers against loss of life or permanent dismemberment while driving, riding, boarding or alighting in an insured vehicle.
Maxi Pro PA Plan is a personal accident insurance plan that provides 24 hours worldwide coverage for bodily injury, disablement or death caused by accidental means. This could include, but not limited to total and permanent disablement, loss of use of one or two limbs and complete and irreversible loss of sight in one or both eyes. This plan also includes accidental death benefit which providing a generous cash lump sum to the insured’s estate in the event of death caused by accident.
Q2: Why do I need these plans?
A2: You need these plans simply because you can never have enough of protection, especially when you need to take care of the welfare of your family and loved ones.
Despite the fact that a person causing an accident cannot assert third-party liability claims against himself or herself, passengers’ accident insurance is extremely important for a driver who would otherwise receive no reparation at all when causing an accident. It is also an important protection in those instances in which it proves impossible to determine the party at fault. Whereas for personal accident insurance, no one can predict when accident will strike, it is better to have protection regardless of how little the protection is than to have nothing at all.
Q3: What is the scope of cover in Maxi Rider PA and Maxi Pro PA?
A3: Maxi Rider PA covers the following benefits:
i) Section I – Loss of Life : RM5,000.00
ii) Section II – Dismemberment
- Loss of both hands or both feet or sight of both eyes : RM5,000.00
- Loss of sight of one eye and one hand/one foot : RM5,000.00
- Loss of one hand or one foot or sight of one eye : RM2,500.00
Maxi Pro PA covers the following benefits:
i) Death : RM25,000.00
ii) Permanent Disablement : RM25,000.00
Benefits are also extended to accidental death or bodily injury caused by:
i) Participation in any sports on an amateur basis not specifically excluded by the Policy;
ii) Strike, riot, civil commotion, hijacking (excluding terrorism risks), murder, assault, disappearance and exposure to elements;
iii) Motor-cycling as a rider or a pillion rider (excluding whilst engaging in racing, pace making speed contest reliability trials);
iv) Woodworking risks, gas inhalation and food & drink poisoning;
v) Harmful insects bites, snake bites and animal bites but excluding illness or disease transmitted by such bites;
vi) Travelling by air as a fare-paying passenger in an aircraft licensed for passenger service on scheduled or unscheduled flights.
Q4: When will the insurance coverage begin and for how long?
A4: Your coverage will begin from the date Liberty Insurance Berhad receives your Application Form and full payment. The policy remains effective for a period of 1 (one) year. Your coverage is valid for as long as you continue to pay premium and/or upon reaching the maximum age limit.
Q5: When does my protection end?
A5: Your policy ends automatically as soon as one of the following happens:
i) the Insured dies;
ii) the Insured reaches 71 years old;
iii) the date on which the Insurer pays benefits which together with any previous payments equals the maximum benefits payable as set out in the policy.
iv) the date You do not pay the annual premium when due;
v) the date You or the Insurer cancels your insurance as set out in the policy.
Q6: If another person caused me an accidental injury, wouldn’t they be responsible to pay for my injuries and loss?
A6: Yes, but accidents are not always caused by another person. In fact, aside from motor accidents, most accidents occur with no one else involved. Why take a chance when you can protect your family’s financial security and your peace of mind with a PA plan.
Q7: If there are changes to my personal profile, must I inform my insurance company?
A7: Yes, you are required to inform your insurance company should there be changes regarding your address, occupation and hazardous personal pursuits which would affect your current risk profile. Otherwise it may affect your claim should there be any undisclosed information arising thereof.
Q8: If I am holding policies from two insurance companies at the same time, can I claim accidental damages under both policies?
A8: Yes, provided that you have declared the insurance coverage at the time of taking out the policy with us or afterwards.
Q9: What about Beneficiary, do I have to submit any names?
A9: Yes, you are advised to nominate a beneficiary or more than one and ensure that he/she is aware of the PA policy that you have purchased. In the event of your disablement or demise, you or your beneficiary will receive the compensation. (Note: Nomination form does not apply for MUSLIMS)
Q10: Can you tell me why it is necessary to have the witness's signature on the nomination form? Can I ignore it or have my family members to sign it?
A10: Yes, it is important to have the witness' signature to prevent future disputes or forgery. Any of your family members can be the witness.
Q1: Who is eligible for this scheme?
A1: For Maxi Rider PA
i) Driver - anyone who resides in Malaysia with a valid driver’s license under the regulations of the Malaysia Road Transport Department
ii) Passenger – anyone in between the ages of 3 to 70
For Maxi Pro PA
i) Anyone who resides in Malaysia between the ages of 3 to 60 is eligible for this plan and it is renewable up to the age of 70.
Q2: Can I buy a joint insurance coverage together with my spouse and/or my children?
A2: For Maxi Rider PA - Yes, you may include your family in this policy as it covers both driver and passengers up to the limit of 5 people.
For Maxi Pro PA - No, this plan is exclusively for individuals. Your spouse and/or children will have to buy a separate policy.
Q3: Can I be insured if I am handicapped from polio or other physical impairment?
A3: No, these plans are designed for people without any pre-existing physical infirmity prior to his/her purchase of the policy.
Q4: Can I take another PA Insurance if I already have one or more PA Insurance with other Insurance Companies?
A4: Yes, you are allowed to take more than one PA Insurance. The benefits are paid in addition to any other insurance plan that you may have elsewhere only for “Death and Permanent disablement”.
Q5: I already have an existing PA with your company. Can I convert this to these PA plans?
A5: Each PA offered by our company has different features, which correspond to the premiums charged. Therefore you cannot convert your existing PA. You may enrol in the either Maxi Rider PA Plan or Maxi Pro PA Plan and alternatively either choose to continue your existing PA until expiry or terminate and opt to receive a refund (short period rate premium).
PAYMENT AND BILLING RELATED
Q1: Can I pay the premium by instalments?
A1: No, because this is an annual premium payment plan with premium as low as RM19.08 only. This premium amount is inclusive of Goods and Services tax at 6%.
Q2: Can I sign up for Auto Renewal with my credit card?
A2: Yes, you can enrol for these plans with Auto Renewal Instruction where we will automatically charge to your Credit Card for renewal of yearly premium.
Q3: How do I enrol for the PA Insurance?
A3: Just fill-up the proposal form and submit it together with the premium payment through our appointed Agents or directly to our Branches nationwide or call Toll Free 1800-888-990.
You may choose any of the following payment options:
- Pay Cash/ Cheque/ Credit Card at UAGIB Branches or to Agents;
- Fax the proposal form to UAGIB Call Centre with details of your Credit Card (Visa or Master);
Q4: Do you provide Maxi PA Scheme for foreign maids?
A4: No. However, we do have Foreign Workers PA Scheme to accommodate foreign maids (Please call our Toll Free 1800-888-990 for more information).
Q5: Do I need to pay stamp duty?
A5: Yes, a stamp duty of RM10 is payable for each new or renewal policy and it shall be included in the first payment.
Q1: How do I make a claim?
A1: In the event of a claim situation arising, notify UAGIB immediately at the following numbers:

Q2: What are the documents my family or I need to complete and submit?
A2: The claimant will need to complete and submit the Claim Form to UAGIB as soon as possible together with the following documents:
- Medical report
- Original medical bills & receipts
- Copy of police report
- Copy of death certificate
- Copy of post-mortem report (if available)
- Burial certificate
- Letters of Administration
Q3: Can I claim from an accident arising from works that happen outside of Malaysia?
A3: Yes, Maxi Pro PA protects you with 24 hours worldwide coverage while you are on short overseas business trips.
Q4: Who from my family is allowed to claim in the event of death occurrence?
A4: Your nominated beneficiaries or Legal Estate is allowed to claim in the event of death occurrence.
Q5: What is “Short Period Rate”?
A5: “Short Period Rate” means the Insured may cancel the policy at any time after receipt of the policy. The policy will be cancelled and the earned premium shall be computed in accordance with the below short rate table used by the company at any time of cancellation.
i) Not more than one month : 25%
ii) More than 1 month but within 3 months : 50%
iii) More than 3 months but within 6 months : 75%
iv) More than 6 months but within 9 months : 90%
v) More than 9 months : 100%
Q1: Do these plans cover death due to natural death and illness?
A1: No, these plans only cover death due to accident.
Q2: Do these plans cover death or TPD arising from Tsunami, natural disasters and/or terrorism?
A2: No, these plans do not cover Tsunamis, Earthquakes, Natural Catastrophes and/or Terrorism.
Q3: Do these plans provide cover for Death or Disability arising from insect bites?
A3: Yes, Maxi Pro PA covers death or disability arising from insect bites (except mosquitoes). Maxi Rider PA only covers death or disability arising from accidental means.
Q4: Is there any exclusion?
A4: Yes, standard PA exclusions apply. Generally, the benefits shall not be paid if the Insured Person sustains bodily injury due to exclusions below:
- If the driver does not hold a valid driver’s license to drive the vehicle or if not qualified for holding or obtaining such a valid driver’s license under the regulation of the Malaysia Road Transport Department (For Maxi Rider PA)
- While the vehicle is used for illegal business as an unlicensed common carrier (For Maxi Rider PA)
- If the vehicle is used for hire or reward, road rally pace making, speed-testing or use for any purpose in connection with the motor trade (For Maxi Rider PA)
- Flying as a pilot or crew member in any aircraft other than as a fare paying passenger in an aircraft licensed for passenger service (For Maxi Pro PA)
- Radioactive and nuclear weapon material accidents (For Maxi Pro PA)
- Natural perils such as earthquake, windstorm, volcanic eruption, floor, lightning, tidal wave, cyclone and typhoon (For Maxi Pro PA)
- Certain hazardous or high risk sporting activities such as mountaineering involving the use of ropes, caving, parachuting, hang-gliding, hunting, racing of any kind (other than on foot), scuba-diving, bungee jumping, water ski jumping or private aviation (For Maxi Pro PA)
- Childbirth, miscarriage, pregnancy or any other complications thereof (Both)
- War or contamination by war, invasion, act of foreign enemy, civil war, mutiny, military uprising, revolution or martial law (Both)
- Bacterial infections, suicide or self-inflicted injury, pre-existing physical or mental defect or infirmity, HIV and AIDS related diseases (Both)
- Criminal acts or terrorism (Both)
- Non-prescribed use of drugs or alcohol (Both)
